Our hospital readmission reduction program is designed to successfully work with hospitals and insurers towards the common goal of preventing hospital readmission. With coaching and support the patient is better equipped to attend doctor visits, have medication management, mobility or transfer assistance. For a minimum of 30 days our certified caregiver supports the care plan after discharge and can alert the appropriate medical staff if any red flags are detected.
Family First Homecare will assist you or your loved one as they transition from one level of care to another. Our care specialist is involved with patients who may be discharging from the hospital to home or home-based community and are in need of assistance while they heal.
This period carries a high risk of medical errors and adverse events due to fragmented communication, or unsafe hand off due to the need to integrate between the hospital organization and home-based care providers. With completion of a needs assessment we implement the care plan non medical component to achieve successful outcomes. Family First Homecare assist in managing and coordinating care and connecting our patients with community resources.

Approximately one in five hospitalized patients are readmitted within 30 days and a third of patients are readmitted within 90 days. Our home-based care program includes 24- hour support or intermittent assistance for frail older adults, patients with disabilities or those in need of prolonged post- hospitalization care. Our care specialist will provide an effective, safe and appropriate continuation of support.
Family First Homecare's overarching goal is to reduce unnecessary hospitalizations and the cost of custodial care. Our home-based care and assistance focus the majority of services toward the chronically ill or functionally frail patient who may be at a higher risk for patient safety events such as medication errors and falls.
